Heading out of town or having a pet sitter step in? Don’t forget to complete our third-party authorization form! This ensures your pet sitter, or any authorized agent can bring your pet to the ER and facilitate a seamless check-in while you’re away. The form also grants them the ability to make emergency veterinary decisions for your pet if you cannot be reached.
