PATIENT TRANSFERS

Animal Emergency & Specialty Center welcomes the transfer of patients to our Emergency and Internal Medicine services, as well as those patients requiring 24-hour care.

EMERGENCY TRANSFERS

If you wish to transfer an emergency patient, we do request that you call to speak with the receiving doctor so that we may prepare for your patient’s arrival: (360) 697-7771

INTERNAL MEDICINE AND SURGERY TRANSFERS

To refer a patient to our Internal Medicine or Surgery service, please fill out the form below or download the referral form and send the form and records to [email protected].

For an efficient transfer, we recommend preparing the following:

• Any lab work – CBC, Chemistry panel, urinalysis, fecal, other – including previous blood work for comparison

• Any biopsy, cytology reports

• Any surgery reports

• All previous diagnostic imaging interpretation reports including radiographs, ultrasound, CT, MRI

• All referral reports if the patient has been seen by a specialist
